Fit is the whole game. A cage that's off by a quarter inch lives in a drawer. Take two minutes now and the recommendation gets a lot sharper.

Flaccid length
Stand up straight. Press a ruler against the pubic bone at the base and lay it along the top of the shaft. Measure from base to tip, soft.

Flaccid shaft girth
Wrap a soft tape around the thickest part of the shaft, soft. This is the circumference, not the width.

Base ring circumference
Wrap the tape all the way around the base of the penis and behind the testicles, where the ring will sit. Snug, not tight. This sets your ring size.
Measure soft, ideally after a warm shower, and take it a few times across the day then average. The gap between ring and cage depends on how your testicles hang: low hangers want a thinner gap, high and tight wants more room.
